Tap the minus and plus on the main screen. You can also change it from your Apple Watch, the Dynamic Island, or the Live Activity on your lock screen, without unlocking.

Does the beat keep going with the screen off?

Yes. Start it, put the phone in your pocket, and the beat holds while the screen is off.

What cadence should I use?

Around 180 steps a minute is a common target, which is where the name comes from. It is a starting point, not a rule. Set it anywhere from 150 to 230 and use what suits you.
